ACL Tear: Do You Need Surgery Immediately? Why Prehab & Proper Rehab Matter More Than You Think

Anterior Cruciate Ligament (ACL) injuries are one of the most feared knee injuries, especially among athletes and active individuals. In Hong Kong, we commonly see ACL tears from football, basketball, rugby, skiing, and high-intensity training such as HYROX or CrossFit.

One of the most common questions patients ask is: “My ACL is torn, do I need surgery straight away?” The short answer is not always. The better answer is, the decision should never be rushed, and physiotherapy plays a critical role before and after surgery.

What Happens When the ACL Tears?

The ACL is a key stabilising ligament in the knee that controls:

  • Forward movement of the tibia

  • Rotational stability

  • Dynamic control during cutting, landing, and pivoting

When the ACL tears, patients may experience:

  • A “pop” sensation at injury

  • Immediate swelling

  • Knee instability or “giving way”

  • Difficulty returning to sport or sudden direction changes

However, pain level alone does NOT determine severity, and many people regain daily function surprisingly quickly, which is why proper assessment is essential.

Do All ACL Tears Require Surgery?

No.

ACL management depends on multiple factors, including:

  • Your sport and activity demands

  • Knee stability

  • Presence of meniscus or cartilage injury

  • Age and lifestyle

  • Ability to commit to structured rehabilitation

Some individuals can function well with high-quality physiotherapy alone, especially if:

  • They do not participate in pivoting sports

  • They regain good strength and neuromuscular control

  • The knee remains stable during functional testing

That said, for athletes involved in cutting, jumping, or contact sports, ACL reconstruction is often recommended to reduce long-term instability and secondary injuries.

The key point: Surgery is a choice! Not an emergency. And good rehab starts BEFORE surgery.

Why Prehab (Pre-Surgery Rehabilitation) Is So Important

Prehab is one of the most overlooked but powerful phases of ACL recovery.

Research consistently shows that patients who undergo structured physiotherapy before ACL surgery experience:

  • Better post-operative strength

  • Faster return of knee range of motion

  • Improved functional outcomes

  • Lower risk of complications

What Does ACL Prehab Focus On?

A proper prehab program includes:

  • Restoring full knee extension and flexion

  • Reducing swelling and pain

  • Strengthening quadriceps, hamstrings, glutes, and calves

  • Improving balance and movement control

  • Preparing the body (and mindset) for surgery

At MOBILIZE, we often say: “Your post-op recovery is only as good as your pre-op preparation.”

ACL Surgery Is Only the Beginning — Not the End

Many patients believe:

“Once I’ve had surgery, I’ll be fine.”

In reality, ACL surgery does not automatically restore knee function. The graft needs time to heal, adapt, and integrate — and rehabilitation is what determines success.

Post-Op ACL Rehab: What Patients Need to Know

1. Early Rehab Is About Control, Not Aggression

The first phase focuses on:

  • Swelling control

  • Pain management

  • Regaining knee extension

  • Activating quadriceps

Doing “too much too soon” can delay recovery.

2. Strength Symmetry Matters More Than Time

Returning to sport should never be based on months alone. Key rehab milestones include:

  • Quadriceps and hamstring strength symmetry

  • Single-leg control

  • Jumping and landing mechanics

  • Change-of-direction ability

Many re-injuries happen not because rehab was “too short”, but because it was incomplete.

3. Psychological Readiness Is Real

Fear of re-injury is common and often underestimated. Good ACL rehab addresses:

  • Confidence in the knee

  • Gradual exposure to sport-specific movements

  • Objective testing to guide safe progression

Why ACL Re-Tears Happen

Even months after surgery, ACL re-injury risk remains if:

  • Strength deficits persist

  • Movement patterns are not corrected

  • Return-to-sport decisions are rushed

  • Rehab lacks sport-specific loading

This is why physio-led, criteria-based rehab is essential — not just generic exercises.

Our Philosophy on ACL Rehab at MOBILIZE

We believe ACL rehabilitation should be:

  • Individualised — not protocol-only

  • Evidence-based — guided by objective testing

  • Progressive — from control → strength → power → sport

  • Honest — no false timelines, no shortcuts

Whether you choose non-surgical management, prehab before surgery, or post-operative rehabilitation, the goal is the same: A strong, confident knee you can trust.
